A unique service that assures seniors receive needed exercise and physical activity essential for functional independence. Regularly performed personalized exercise is: preventive medicine, it promotes "aging in place," and provides a money-saving option by keeping seniors independent and in their own home in the Portland/Beaverton area. Aging and staying independent requires strength, mobility, balance, joint stability, range of motion and flexibility. With a senior/geriatric Home Fitness Care Plan we bring wellness to the individual at home. The prescribed exercise program helps ease performing activities of daily living, improve circulation, enhance strength, enhance range of motion and balance. This reduces the risk of falls and fractures, and thus reduces the risk of disability and dependency. Personalized exercise programs promote overall health and independence, and we bring it with compassion and care.
What is Home Fitness Care? It is Exercise with Compassion. A "Home Senior/Geriatric Fitness Care Plan" consists of once, twice, to three times a week or more of supervised geriatric exercise performed at a safe level of effort while taking into account chronic medical conditions. A full range of good geriatric fitness and therapeutic exercise services are available to enhance: circulation, range of motion, mobility and gait; restoring muscle strength and balance after an illness or bedrest; walking, balance and flexibility exercises. The personalized exercises are performed under careful guidance of a health fitness professional. Checking blood pressure and heart rate are often performed as part of the fitness care sessions. We provide progress reports to family members and healthcare providers upon request. In the event of an observed change in behavior, change in physical function, vital signs or medical condition, these will be responded to appropriately.
Fitness Care at home in the community: By rebuilding and maintaining strength, balance and mobility seniors can continue to enjoy living independently at home. With regular Home Fitness Care seniors can extend longevity with better health and function, and reduce the risk of falls and fractures.
Fitness Care in assisted living or a adult foster home: Home Fitness Care helps maintain and improve functional health; a battery of regularly performed exercises helps slows down the rate of physical decline, reduces and prevents the risk of falls and fractures. Fitness Care can reverse and reduce the increasing need for assistance in personal care and thus reduce long-term health care costs.
Therapeutic Exercise and Post-Rehabilitation: Our fitness care professionals can assist in carrying out instruction of the health care provider while providing support and guidance to the individual, from continuingphysical therapy recommended exercise, therapeutic exercise to daily walking and balance exercises. We provide a safe and effective bridge for our clients to cross from the structured clinical treatment to mainstream community or home-based exercise and we do it with compassion and care. We work with individuals following treatment and/or rehabilitation for clinically documented chronic disease, musculoskeletal injury and/or disabilities.
Extending Independence: A sedentary lifestyle of too much sitting and long-term inactivity because of an illness or bed rest causes loss of muscle strength and weight gain (sarcopenic obesity), loss of balance and reduces the ability to carry out activities of daily living, this places one at a higher risk of falls, fractures, disability and dependency which is typical of the geriatric syndrome. Breaking the downward spiral is possible with research and evidence based exercises that benefit activities of daily living by enhancing muscle strength, improving joint stability, balance, posture and mobility, thereby extending independence.
Fitness Care Professionals: Our fitness care professionals hold nationally recognized certifications from ACSM or ACE as Health & Fitness Specialist, Clinical Exercise Specialist and Personal Trainer and have extensive experience in older adult exercise and exercise for individuals with chronic medical conditions, illness and disabilities, and have a current First Aid, CPR, AED certificate.
