We specialize in helping aging adults in their 60’s, 70’s, 80’s and BEYOND, stay active and vibrant, maintain independence, with better function, less pain and with a higher quality of life.

Our in-home outpatient physical therapy supports the scientifically proven connection between restoring and building strength, and active aging. In-home Outpatient Physical Therapy is convenient, time efficient, less stressful to the patient and/or caregiver. We can usually schedule you for evaluation the same week that we receive your phone call and begin receiving treatment.

We would be honored to work with you and become YOUR concierge physical therapist! Our in-home evaluation as well as physical therapy treatment sessions are 60 minutes (at most clinics this is only 40 minutes). Fitness & Function is a Fee for Service company, some of our PT’s accept clients with Medicare B.  

Our licensed Physical Therapist Bayla is a specialist in the field of geriatric physical therapy, active aging, senior fitness and wellness
Bayla is the first physical therapist in Oregon to achieve certification is all three of the CDC’s senior fall reduction programs: Tai Chi Movement for Better Balance, Stepping On, and Otago. Bayla can except clients with Medicare B, and sees clients in Beaverton, Hillsboro, NW Portland, Lake Oswego and the surrounding areas.

Lisa’s work as a physical therapist has spanned over 20 years and has included a variety of work environments ranging from inpatient hospital settings, home health settings, orthopedic outpatient clinics, work conditioning and hardening clinics, and industrial settings. Lisa’s expertise includes management strategies for chronic orthopedic issues such as arthritis, education and exercise programs to minimize the risk for falls, instruction and recommendations for assistive devices such as canes and walkers, and general exercise programs to maintain maximum independence and health within the community. Lisa accepts clients in Clackamas and NE Portland and surrounding areas for in-home personal training and outpatient physical therapy ranging in age from teens up into seniors into their nineties.

Oregon State law provides Direct Access to licensed physical therapists without a physician’s referral or prescription. This means that you can seek medical treatment for your musculoskeletal issues directly from the physical therapist of your choosing. This places physical therapy as a first line provider for your musculoskeletal health.

  • Convenience: Thirty percent of seniors never follow up on their doctor’s recommendation for physical therapy and the issue does not get resolved.
    • It often takes too much time and effort to get ready for outings when a person is in pain, has difficulty with balance or is worried about falling.
    • It is often difficult and seen as a burden to ask a family member to rearrange their day or take time off from their job to take you to multiple therapy appointments.
    • One clinic appointment can often take 3-4 hours of time for both the older adult and caregiver!
  • No waiting lists: when you are injured and in pain, your quality of life is affected and you want treatment to start as soon as possible. We generally can schedule your evaluation the same week that you call us and begin treatment soon after, so that you can resume your life.
  • Less stress for patient and caregiver: In-home outpatient physical therapy provides less stress for the patient; no transportation to arrange, don’t have get ready for the trip to the clinic, by the time the patient reaches the clinic he/she is already tired and stressed. The in-home treatment time provides a break for the caregiver and reduces caregiver stress.  
  • Better learning environment:
    • It is often difficult to transfer and incorporate the skills learned in a clinic set-up to how one should move about in their own home.
      Physical Therapy in-home allows the client to learn and incorporate strength and balance skills safely while he or she moves throughout their own home and neighborhood.
    • The client learns how to rise from different types of furniture, their own toilet, to move around in their kitchen and reach into high and low cupboards, get in and out of the shower, navigating through front -back-garage doors, up/down stairs, and walking on their own drive way, side walk or safely through their backyard while gardening.

Q. What does Direct Access mean for you? Immediate evaluation, diagnosis and treatment from your Physical Therapist for your musculoskeletal injuries without the delay in visiting another health-care professional.  Immediate consultation with your Physical Therapist to determine if therapy is needed or if further examination by another health-care practitioner is warranted

Q.  Is a Physician’s referral needed for insurance to pay? State laws allow consumers to access physical therapy services directly, without the need of a physician’s referral. Insurance reimbursement and the requirement of a physician’s referral, however, is plan-specific. Most insurances do not require a referral, but some do. If you do not have a physician’s referral for physical therapy and are uncertain if one is required in order for insurance to pay, we recommend that you contact your insurance company directly to verify this.

Q. What is you Appointment Cancellation Policy? If you should need to cancel an appointment, please be aware of our requirement of at least 24-hour notice. Failure to cancel your appointment with more than 24-hour notice will result in a $25 fee. Insurance will not pay for this fee, so you will be responsible to pay at your next visit.

Q. Do you take insurance?  Fitness & Function’s Physical Therapy services are a designated ‘out of network’ provider for clients who have commercial insurance plans with benefit coverage for physical therapy. Most insurance plans coverage for out of network is 60% to 80% of charges. It is the client’s responsibility to know their insurance plan and coverage. Fitness & Function Physical Therapy services is Fee for Service business model; all charges must be paid on the same day of service. Clients are provided with a receipt/bill that includes all appropriate billing codes to submit to their insurance for reimbursement.

Q. Are you an Approved Medicare Provider? We are approved non-participating Medicare B provider. Fitness & Function accepts payment in full from the client/patient at the time of service. Fitness & Function sends your claim for covered services to Medicare. The client/patient receives reimbursement from Medicare for approved services. In most cases there are only small differences in overall out of pocket cost to our clients that have either/both a commercial or Medicare insurance plan(s). Reimbursement varies depending on the insurance coverage..

Q. Are all Physical Therapy sessions performed by the physical therapist?  Yes, Evaluation and Treatment sessions are 60 minutes exclusively with our licensed Physical Therapist; clients do not receive services from a PT aide or PT assistant.

Q. Why are you a Fee for Services company? The fee for service outpatient physical therapy allows our physical therapist to deliver services independent of the restrictive rules and regulations and high cost associated with being an in-network provider.

  1. In today’s current healthcare environment, there exist complex mathematical equations to determine every aspect and cost associated with care provided to a client. Reduced payment and reimbursements are the norm for the industry.
  2. In addition to the fee structure complexity, there is also a lengthy submission-denial-resubmission process before companies are fully compensated for their services.
  3. Most in the healthcare industry will artificially inflate their fees to compensate for reduced reimbursement rates and the associated increased administrative cost.
  4. ther cost containment methods include reducing appointment time and/or utilizing assistants to provide care.
  5. A cash based/fee for service business model allows us to keep our fees/costs below industry norms while still allowing clients to benefit from insurance coverage.
